Vulval Toileting of a Puerperal Client

Vulval toileting cleans the external perineal area after birth while protecting healing tissue, reducing contamination, and promoting comfort. It is performed with consent, privacy, clean technique, and inspection for abnormal bleeding or infection.

Purpose

  • Promote perineal hygiene and comfort
  • Reduce contamination of wounds or perineal tissues
  • Assess healing, lochia, pain, and abnormal findings

Indications

  • Routine postpartum perineal care
  • After urination or bowel movement
  • Perineal tear, episiotomy, heavy lochia, or discomfort

Equipment

  • Privacy, drapes, and suitable position
  • Warm clean water or approved solution
  • Clean or sterile swabs as indicated
  • Gloves, pad, waste receptacle, and hand-hygiene supplies
  • Postnatal record

Preparation

  • Offer analgesia or privacy measures and explain each step.
  • Gather all supplies before exposing the woman.

Procedure steps

  • Explain the care, obtain consent, assess pain, bleeding, wound, urination, and need for analgesia.
  • Provide privacy and position the woman comfortably with adequate support.
  • Perform hand hygiene and wear gloves; inspect the vulva, perineum, wound, lochia, swelling, and discharge.
  • Clean from least contaminated to most contaminated area, front to back, using one swab or clean area once.
  • Avoid pressure on sutures and do not insert anything into the vagina unless clinically indicated.
  • Rinse or dry according to policy, apply a clean pad, and help the woman reposition.
  • Dispose of materials, remove gloves, and perform hand hygiene.
  • Explain self-care, pad changing, handwashing, hydration, and warning signs.
  • Escalate heavy bleeding, wound separation, haematoma, severe pain, fever, offensive lochia, or urinary retention.
  • Document findings, care, pain, education, response, and escalation.

Precautions

  • Maintain dignity and use consent for intimate care.
  • Clean front to back and avoid vigorous rubbing or irritant products.
  • Use aseptic care for fresh or infected wounds according to policy.
  • Assess rather than mask abnormal bleeding, infection, or wound complications.

Aftercare

  • Reassess comfort, bleeding, wound, and voiding.
  • Arrange clinical review for abnormal findings.

Nursing considerations

  • Support independence while respecting cultural practices that are safe.
  • Combine hygiene with postpartum assessment.

Patient education

  • Wash hands, clean front to back, change pads regularly, keep the area dry, and report worsening symptoms.
  • Seek care for heavy bleeding, fever, offensive discharge, wound opening, or severe pain.

Documentation

  • Record wound and lochia findings, care performed, pain, products used, education, and referrals.
  • Document the woman’s response.
